Living Will Declaration

LIVING WILL DECLARATION: is a basic statutory form that expresses your wishes to have your life prolonged by the administration of extraordinary life sustaining procedures when you are unable to participate in the decisions regarding your own health care due to incapacity and you are suffering from an irreversible illness, accident, or condition that will cause my death within a relatively short time.

PLEASE: fill in your legal name. Date the document. Sign in front of two witnesses who observe you sign and then sign their names providing their names and addresses.

There are many other important estate planning devices that should be discussed in the context of a living will. Nonetheless we are including this form so that it is available on an emergency basis when there is no time to consult with an attorney. Our office does offer a small estate plan which starts at $125.00 which includes a will, living will, health care power of attorney, power of attorney, instructions and signing ceremony all in compliance with Montana Law.
